Follow these instructions if this is your second or third consecutive blue ribbon. 

If this is your 4th consecutive award, follow the instructions as a first-time applicant.

Payment is available through the online IAHPERD portal or a check for $150.00 made out to IAHPERD and sent to: Marilyn Bloechle, 1030 Shedron Way, Lombard, IL  60148.

When applying for a blue-ribbon renewal, complete the online application and all tabs under the application section.  (If there are significant changes in the past five years, the blue-ribbon committee may request more information and a possible site visit. )

If your program does not meet the criteria for renewal, the next time you apply, you follow the instructions as a first-time applicant.

Tabs under the document section that need to be completed include:

• Program analysis tab:

  • Your original report should be saved on this application.  

  • Using this original analysis as a basis for information, discuss changes/improvements you have made over the past five years. 

  • Address the recommendations suggested by the Blue Ribbon Team on your previous award. 

•  Two Letters of support from administrators

• The roster of teachers (only include those that teach health) 

• Professional development (activities for the department members,) 

• School Report Card  

• Student work (5 various examples from 5 separate skills)
